Please complete the Name Change form and submit additional documents showing proof of your name change.
The Name Change form can be found on our website by following the steps below:
- Visit our website at www.AmericasChristianCU.com
- On the menu, bar hover over Services
- Under the column Tools & Resources, select Forms & Documents
- Scroll to the Personal Membership section, and then to the sub-section Other
- Select the form titled Name Change

You will also need to provide proof of your name change with an additional document such as a social security card, marriage license, or divorce decree.
We offer two ways to complete and submit forms:
1) Online: A new tab on your internet browser will open to our secure application portal. You may complete the form using a e-signature and submit the document directly to us. A member representative will be in contact with you to fulfill your request.
2) Print: A new tab on your internet browsers will open a PDF. You may print, complete and sign this form then fax or mail it to America’s Christian CU’s headquarters. A member representative will be in contact with you to fulfill your request.
